Membership cancellation

We’re sorry to see you go! Please complete the form below to request the cancellation of your membership.

As a reminder, all Studio Pilates Nashville memberships require 30 days’ notice to process cancellations, as stated in your signed membership agreement. Once this form is submitted, your membership will remain active for 30 days from the date of submission, and you will continue to have access to classes during that period. No refunds will be issued for unused classes, and membership credits do not roll over after cancellation.
